Requirements:

Active and current state license or certification

Minimum of 1 year of clinical experience in the respective field

Excellent communication and interpersonal skills

Ability to work in a fast-paced environment and adapt to changing situations

Strong problem-solving skills and attention to detail

CPR certification required

The physical therapist is responsible for providing skilled physical therapy services, on an intermittent basis, to patients, in their homes and Assisted Living Facilities, following physician orders. The therapist will provide an initial evaluation, and develop a care plan. 

Primary Duties

• Initiates care, evaluates patients' physical therapy needs, and follows up with patients at home. Formulating treatment plans, and making recommendations for types of equipment and discharge plans.

• Maintaining patient treatment schedules according to the patient's needs and conducting family training as necessary.

• Maintain contact with the person most responsible for managing the patient, keeping them informed of any additional patient needs, treatment modifications, and progress.

• Ability to record patient information and coordinate care using EHR software.


Knowledge, Skills, and Ability Qualifications:

• Degree from an accredited Physical Therapy program.

• Current and unrestricted Physical Therapy license in the state where services are rendered.

• Current CPR certification.

• Strong organizational and communication skills.
